You have the right to inspect and copy records, request an amendment or restriction, receive an accounting of disclosures, request confidential communications, and file a complaint.

We may amend this notice. A current notice will be available from the pharmacy. To file a complaint, contact us at 818-343-5100 or info@mycarepluspharmacy.com. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.
